fix: Keep LSP references readable with selection colors

Fixes #9. Use selection foreground/background colors for LSP reference highlights, preserving bold write references. Adds nine regression cases; all nine and six existing tests pass in headless Neovim 0.12.5. Independent review passed. Neovim 0.8 and live LSP rendering remain untested
