SetupCommitFileQueueW looped on a copy failure, prompting for the same media forever when the callback does not supply a new path; now anything but FILEOP_NEWPATH goes to the copy-error branch. Idea from a published patch of another iOS port. MADEIRA_SETUPAPI_MEDIA_FIX=0 restores the loop.
